Fees for services are based on an hourly rate. No insurance is accepted currently. Documentation to file for insurance claims will be provided.
Fee structure: $350.00 flat rate for initial visit up to 90 mins. Subsequent visits will be billed at an hourly rate of $300.00

Reduction of polypharmacy and plan for describing
If you are seeking a thorough review of medications with a proposal for redaction, then this service is for you. Elderly patients are more likely to be impacted by polypharmacy. Providers prescribe medications and often do not have time to consider describing. This service aims to specifically assist in reduction in medication use.
What to expect:
- Physician will review medications, discuss them with you exploring options for a plan for reduction in number of meds, frequency of use or dosage reduction.
- Not all medications are the same, many are ill fit for the wisdom-filled population (those with mileage or age), therefore, a special attention must be lent to reconciling medications for safety in the elderly population.
Advanced care planning, including MOST (medical orders for scope of treatment) form review/completion.
The wisdom-filled population accounts for many ill-advised admissions to hospital, futile care interventions and undignified care towards the end of life. This service is to help people navigate through the dense and emotional discussion of end-of-life care in a proactive manner. Studies have shown patient-provider engagement focused on goals of care to reduce unnecessary health care costs and improve patient satisfaction.
What to expect:
- Physician will engage in a goals of care discussion utilizing DNR and MOST forms to capture your desires and objectives.
Review of hospitalization records
Have you ever been to the hospital and be discharged without fully understanding what was done to you, for you, and what things were changed? Most elderly patients do not understand fully what details of their care are impacted in the aftermath of being admitted to the hospital. This is true even after a follow up visit with your primary care physician due to lack of continuity. If you wish to have a geriatrician review your medical records to provide a summary and answers questions to further enlighten you, then this service is for you.
What to expect:
- Physician will review recent hospital records and provide information about what was discovered and what changes were imposed. A discussion will follow allowing for improved understanding, decision making and care.
Review of nursing home records

- This is a service for those who are currently residing in a nursing home or who have loved ones residing in a nursing home seeking review of records or explore possibilities for improvement in care.
